Client Information, Agreement, Class Rules, and Waivers
NO SHOWS AND RESCHEDULINGS
We understand that scheduling conflicts happen. If you cannot attend any of the offered classes in a particular week, you’ll receive the written material for that week and you’ll be offered a time, IF AVAILABLE, to make up your lesson. Otherwise or on the second occasion of such no show or needed rescheduling, you’ll be moved to a different class to complete the rest of your training curriculum.
TERMINATION OF TRAINING
The class fee must be paid, in full, prior to the class start date. The owner of the dog(s) may have the animal withdrawn from training at any time, however, once training has begun; all fees are nonrefundable.
As the legal owner/agent of the abovementioned pet(s), having carefully read and fully understand this agreement, do hereby waive and release ROSIE DANE DOG TRAINING from any and all liability of any nature. This includes any injury, death, sickness or damage my pet may suffer during or after any training program. I also agree to indemnify and hold harmless ROSIE DANE DOG TRAINING from any and all claims due to damage the pet may cause to any family members of any third parties during or after training. The owner specifically acknowledges that they recognize the risk of taking a group class with other people and their dogs. This risk is entered into with the owner 100% responsible for their dog’s behavior at all times in the class environment.
As the legal owner/agent of the pet(s) noted in this agreement, do hereby state the following information is true and complete to the best of my knowledge. I understand ROSIE DANE DOG TRAINING may need to utilize some or all of this information during the dog(s) training program and I have taken special care to present the information in an accurate fashion.

LICENSES AND VACCINATIONS
§ 91.020 DOG, CAT, AND FERRET LICENSES.
(A) Effective upon passage of this chapter and from that day forward, every owner of an animal who resides or whose animal resides in Jefferson County and who is regulated by this chapter shall apply to MAS or a license facility designated by the Mayor, for any appropriate license required by this chapter. Individual dog, cat, and ferret licenses shall be issued in conjunction with a valid rabies vaccination certificate and shall be valid for the term of the vaccination. License tags and rabies vaccination tags shall be required for each dog or cat four months of age or older. License and vaccination tags shall be firmly attached to a harness or collar worn by the animal.
(B) Animals kept temporarily (not in excess of 30 days unless granted an extension by MAS) for the purpose of breeding or showing shall not require an individual dog or cat license provided the owner has proof of a current rabies vaccination.
